2. Open the Subscription Form, complete the form and submit it to us by e-mail or fax (instructions on the form).

3. Once we receive the order we will contact you to get user access information (names, e-mail addresses, phone numbers).

4. Your log-in information will be e-mailed to you and your subscription will begin once payment has been received.
